All new patients need to be registered into the practice and the patient portal. Please call the scheduling and information line at 612-999-1908 to make an appointment and to register.
Please have the following information ready when calling: Insurance information, date of accident if applicable, Case manager/claims manager name & contact information if applicable.
We will send you an email inviting you to register for the patient portal. Please note that this is mandatory for you to be able to see Dr. Lockman. Intake and permission paperwork is in the patient portal. It is also the only process for video telehealth visits. If you cannot use the portal, please call us.
You can sign into the patient portal directly once you have activated your account.
Inside the portal, you can send messages to Dr. Lockman, make appointment requests, complete forms and have video appointments.
